A questionnaire is a research instrument consisting of a series of questions (or other types of prompts) for the purpose of gathering information from respondents. Questionnaires are also sharply limited by the fact that respondents must be able to read the questions and respond to them.

What makes a good questionnaire?

A good questionnaire is one which help directly achieve the research objectives, provides complete and accurate information; is easy for both interviewers and respondents to complete, is so designed as to make sound analysis and interpretation possible and is brief.

Is questionnaire qualitative or quantitative?

Surveys (questionnaires) can often contain both quantitative and qualitative questions. The quantitative questions might take the form of yes/no, or rating scale (1 to 5), whereas the qualitative questions would present a box where people can write in their own words.

How are questionnaire responses related to data capture?

Questionnaire responses record specifics about data capture – exactly what questions were asked, in what order, what answers were given, etc. Each of these questions is part of the Questionnaire, and as such the Questionnaire is a separately identifiable Resource, whereas the individual questions are not.

How is a questionnaireresponse generated in FHIR?

Each time a questionnaire is completed for a different subject or at a different time, a distinct QuestionnaireResponse is generated, though it may be possible for a previously entered set of answers to be edited or updated.

Can a student see their own answer to a questionnaire?

If the questionnaire was set as “anonymous responses”, the ID and Username will be empty and the Full name field will display “Anonymous”. On the Questionnaire Report page, users with the necessary permissions can view their own responses on the page entitled “Your results”. Usually students will be allowed to view their own responses.
